1. ID Proof/ Address Proof is not clear and self-attested.
2. The address mentioned in the KYC does not match with the proof.
3. Overwriting in the form.
4. Application form is not completely printed/ attached/ filled
5. Signatures are missing on a few pages.
6. Signatures are mismatched in the form.
7. Signatures are not done in appropriate field.
8. Photograph is not affixed in the form.
9. Relationship with the nominee is not mentioned.
10. Minor is added as nominee.
11. Multiple nominees added without declaring percentage.
